2024 Women’s Retreat - Youth Waiver
Please review, sign, and submit this form if a minor child will be accompanying you to the event. Parent/guardian consent is required for all participants under 18.
As the parent or guardian of a minor child/children planning to attend this event, I acknowledge and agree to the following:
Minor participants are not allowed to occupy (overnight in) a room at the event without at least one adult present.
While Christ Community Church will do everything it can to ensure safety, it accepts no liability for accidents or injuries that occur during the event, including those that occur on hotel premises (e.g., rooms, pool, dining areas).
